Making a Referral

We are the commissioned provider for the south east region and can accept referrals from the following places:

  • BLMK
  • Hertfordshire & West Essex (West Essex Tier 4 only)
  • Buckinghamshire (Tier 4 only)
  • Norfolk and Waveney (Tier 4 only)
  • Cambridgeshire & Peterborough (Tier 4 only)
  • Oxfordshire
  • Northampton (Tier 4 only)
  • Suffolk (Tier 4 only) & North East Essex

All NHS GP referrals must be submitted using the NHS e-Referral Service (e-RS). We are not able to accept referrals sent by email or post.

Please read exclusion criteria on our referral form and on e-RS before referring patients.

PLEASE ENSURE YOU COMPLETE THE BARIATRIC PRO-FORMA AND ATTACH TO THE REFERRAL ON E-RS. WE WILL BE UNABLE TO ACCEPT REFERRALS WITHOUT A FULLY COMPLETED PRO-FORMA ATTACHED.

What information must be included?

In order to triage the referral appropriately and to ensure that we are the right service for the patient, we must be able to triage the referral against the access criteria. We have collaborated with BLMK ICS and primary care practitioners to design a referral form for use on e-RS. Please complete all fields on the referral form and send to us via e-RS as our service is unable to accept ‘choose and book’ appointments.

Please highlight on the referral if a patient:

  • has already had bariatric surgery
  • has a significant learning difficulty
  • needs an interpreter
  • history of or currently under a mental health service

Please note that incomplete referrals or for patients that do not meet the access criteria will be sent back to the referrer.

Advice and guidance

Following our move to the NHS e-Referral Service (e-RS), we are now able to accept advice and guidance requests relating to bariatric patients. We are no longer able to accept advice and guidance requests sent by email or post.
