Diabetes
Specialist Multidisciplinary teams provide general and specialist diabetes care for local patients in the inpatient and outpatient setting. The adult diabetes service is for any patient with type 1 diabetes and for patients with type 2 diabetes whose GP recommends a specialist review.
In conjunction with the paediatric team, we offer a transition clinic for patients aged 16-19 years, before being transferred into a young adult clinic for patients aged 19-25 years
We have an Integrated Community Diabetes team who supports patients with diabetes under the care of their GP.
There is a team of educators who deliver structured education in the community to patients with Type 2 diabetes.
The clinical team consists of Diabetes Consultants, Nurse Consultants, Diabetes Specialist Nurses, Diabetes Specialist Dietitians and Podiatrists and a Clinical Psychologist. We are supported by a team of admin staff and clinic support staff.
