Early Self-Referral to Maternity Services
Are you pregnant and wanting to have your baby at one of our hospitals? You can make a self-referral for antenatal care at either of our hospital sites, as soon as you find out you are pregnant so that we can start your antenatal care. You don’t need to speak to your GP first.
In order that we can make your first appointment for you to meet us, please choose which hospital you would like to attend and complete your details on the form. We will then get back to you with an appointment day and time.
Once the form has been completed it will be sent to the Community Midwifery Services and the team will be in touch with you, on the number you have provided.
Please contact us if you have not heard from us within 15 working days. If you are more than 9 weeks, please contact us as soon as you have submitted your referral form so we can process your referral and book you an appointment as soon as possible.
